Reporting Information

Florida Department of Health in Flagler County Epidemiology

All licensed practitioners, hospitals, and laboratories are required to report diseases of public health significance per Florida Administrative Code Chapter 64D-3 and Florida Statute Chapter 381.0031.

The Physician/Hospital Report of Communicable Diseases is the form used to report communicable diseases. Please complete the form and fax or mail it to our office with a copy of the lab report and patient demographics. You can find the latest forms and information on the Florida Department of Health website.

All animal bites must be reported to DOH-Flagler so we can conduct a rabies risk assessment. We work closely in conjunction with the animal control agencies in Flagler County to assure proper management of the offending animal. Please complete the Rabies Exposure Form and fax it to our office at 386-437-8287.

There are other communicable diseases such as tuberculosis, sexually transmitted diseases, and HIV/AIDS that are covered under Chapter 64D-3. If you want to know where to send a report on any of the reportable diseases and conditions, please report these diseases to their respective programs.
