Immunizations reduce absences both at daycare, school and other activities and decrease the spread of illness at home and in the community.
Immunizations for children are always free at the Florida Department of Health in Flagler County. Prior to enrollment into a childcare facility, or to attend school (kindergarten through 12th grade) children are required to have a Florida Certificate of Immunization (DH 680 form, a.k.a. "blue card").

Before students enter the 7th grade, proof of current immunizations and additional booster is required:
Appointments
Appointments are required for child immunizations. Contact our Appointment Line 386-437-7350, choose Option 3, then Option 4 for Immunizations, to make, change or cancel appointment for your child. Customer service representatives are available to assist clients 8 a.m. to 4:30 p.m.
Records
Transferring students in need of an official immunization record (DOH Form 680) must provide their immunization record from their previous provider. Records may be submitted in advance by:
