Florida Department of Health in Flagler County

The Florida Department of Health in Flagler County is a Vaccine for Children (VFC) provider. We give all shots necessary for Childcare centers and schools. DOH Form 680 ("Blue Cards") are available upon request. Religious Exemptions for immunizations (DOH Form 681) are available through our Immunization Department.

COVID-19 vaccinations are not covered by the Department of Health’s Religious Exemption From Immunization (DH 681) Form. The Flagler Health Department recommends that all students ages 5 and older are vaccinated to protect them from this virus. While this vaccination is not required, unvaccinated students cause a risk to themselves and the overall health of a school community.

The Immunization Center provides:

  • Routine infant & childhood shots for free 

  • Back to School immunizations for free through high school

  • Tetanus and pneumonia shots

  • Routine adult vaccines - call for fees and availability

We offer childhood vaccinations from birth to 18 years of age at no cost. Free flu shots are also available for children.

The clinic is open weekdays by appointment only. Parents should bring shot records, photo ID/driver's license and insurance card. Please call 386-437-7350 ext. 3111 for scheduling and additional information.
